Job Description | Provide direct care services in the private homes of individuals with Intellectual Developmental Disabilities and/or other disabilities. \\Provide training and/or assistance with activities of daily living, to include but not limited to bathing, toileting, preparing meals, hygiene, housekeeping, etc. \\Coordinate appointments. \Accompany community outings/events/recreational activities. Implements the Individual Service Plans (ISP). \\Assure health and safety in the home and/or community. \\Make recommendations based on the participant’s needs. \Completes documentation. |

Skills and Experience | One to six months’ experience working with persons with disabilities in a group home, day support, hospital, treatment center, community, or nursing home setting (preferred). \Ø Knowledge of Intellectual Developmental Disabilities (IDD), mental health and/or other developmental disabilities\ Ø Demonstrated skill in working with adults and/or children with disabilities. \\Ø Ability to observe the rights and personal dignity of participants served. Ø Good oral and written communication skills. Ø Ability to perform simple math skills. \Ø Ability to implement and follow policies and procedures. \Ø Ability to participate in meetings with other professionals. \Ø Knowledge of simple nursing care, first aid, behavior management, personal care, hygiene, and environmental hygiene. \Ø Ability to teach participants eating, bathing, dressing, grooming and other self-care skills. \Ø Ability to participate in treatment planning, implement ISP and complete proper documentation. \Ø Ability to observe, record and properly report participant’s behavior, attitude, physical condition, and emergency incident. \Ø Ability to maintain effective working relationships with participants, family members and professional contacts. |
