    Job DescriptionThe Registered Nurse (RN) supports the clinical assessment and care planning needs of clients receiving personal care services within a home care setting. This role focuses on conducting client assessments, developing and maintaining Plans of Services, performing supervisory visits, and ensuring compliance with agency policies and Virginia home care regulations. The RN works independently while supporting quality care, communication, documentation, and continuity of services for clients in the community.
    Key ResponsibilitiesConduct initial and ongoing in-home client assessments to determine eligibility and appropriateness for personal care services. Develop, review, and maintain Plans of Services in collaboration with clients, families, and the care team. Perform required supervisory visits, monitor client progress, maintain accurate and timely documentation, and provide guidance related to client care concerns as needed. Ensure compliance with state regulations, agency policies, confidentiality standards, and documentation requirements while supporting quality care delivery in the home setting.

    Skills and ExperienceCandidates must possess an active and unrestricted Virginia Registered Nurse (RN) license with a minimum of one (1) year of nursing experience, preferably in home care, home health, case management, or community-based services. The ideal candidate should demonstrate strong assessment, documentation, organization, communication, and problem-solving skills. Applicants must be comfortable working independently, conducting in-home visits, maintaining compliance with care plans and regulatory standards, and communicating effectively with clients, caregivers, families, and agency leadership. Reliable transportation and the ability to manage documentation and follow-up responsibilities in a timely manner are required.
